Principle

Collect time-stamped, structured information about exposure, symptom onset, physiologic parameters, treatments (including timing and dose of epinephrine), patient response, disposition, and follow-up recommendations to ensure continuity and accountability.

Demonstration

A single-page template includes fields for: time of exposure and symptom onset; presenting signs (airway, breathing, circulation, skin, GI, neurologic); baseline and subsequent vitals with timestamps; doses and routes of epinephrine, oxygen and IV fluids; response at defined intervals; consultations requested; patient education and prescription of autoinjector; and disposition note with observation plan.

Misapplication

Using only free-text notes without time stamps or omitting epinephrine timing, which obscures the sequence of care and impairs quality review or legal clarity.
